Acknowledgment of Risk and Emergency Consent
I am fully aware of and appreciate the risks, including the risk of catastrophic injury, paralysis, and even death, as well as other damages and losses, associated with participation in gymnastics. I hereby release Orange Kids Gymnastics, LLC from all liability due to injuries relating to these activities.
In the event of a serious injury where parents, guardians, or the child’s physician cannot be reached, I authorize Orange Kids Gymnastics, LLC and its agents to select emergency medical care as needed.
I also give my permission for photos and videos of my child taken during classes to be used for social media and promotional purposes.
Tuition and Membership Cancellation Policy
Billing Date:
Tuition is charged on the day a student enrolls in a class and will be automatically billed on the same date each following month.
(For example, if the enrollment date is April 23, the next charges will occur on May 23, June 23, etc.)
Membership Cancellation:
To cancel membership and avoid the next automatic payment, you must notify us at least 30 days before your next billing date. Notification must be sent by email or text message.
If the cancellation notice is received less than 30 days before the next billing date, the tuition for the upcoming month will still be charged, and the student’s spot in the class will remain reserved until the end of the paid period, regardless of attendance.
For example: If your billing date is the 15th of each month and you notify us of cancellation on May 10, the payment on May 15 will still be processed for the following month (until June 15). The student’s spot in the class will remain reserved until June 15, regardless of actual attendance.
Non-Refundable Tuition:
All tuition payments are non-refundable. No refunds will be issued, even if the student does not attend the remaining classes. The only exception is in cases of billing system error.
Card Payment Fee:
A 3% processing fee applies to all credit/debit card payments.
Make-Up Class Policy
Missed classes may be made up within the same calendar month only, subject to class availability.
Make-up classes are not guaranteed and must be scheduled in advance.
Unused make-up classes do not carry over to the next month and are not refundable.
